From the Madigan Army Medical Center Gate (Interstate 5, Exit 122) continue on Jackson Ave. Jackson Ave become Pendleton Ave. Make a left on 41st Division Drive. The Allen Soldier Centered Medical Home (SCMH) offers a broad range of primary and specialty care services to active duty soldiers of the 1-2 SBCT.

Providers at the Allen SCMH are available for non-urgent medical consultation using the MHS GENESIS Patient Portal. Soldiers are encouraged to enroll in the MHS GENESIS Patient Portal to

  • Discuss your current health and determine if you need to make an appointment
  • Discuss an existing or expiring profile
  • Request lab and radiology results
  • Request prescription refills

Sick Call

  • You must be screened by your battalion and receive a sick call slip.
  • If you need to be seen by a provider for sick call, report to the Allen SCMH between 6:30-7:30 a.m.
  • If you do not need to see a provider during sick call, but need to schedule an appointment, please call the Puget Sound Military Appointment Center at (800) 404-4506.

Behavioral Health

  • Behavioral Health services are available for Soldiers at the Allen SCMH
  • Behavioral Health services are also available for Soldiers at 1-2 SBCT Embedded Behavioral Health (EBH) located at R3742 Railroad Ave, JBLM, WA 98433.

Physicals

  • All chapter, school and ETS physicals will be scheduled at the Allen SCMH
  • Physical packets can be picked up at the Allen SCMH
  • Soldiers must complete all paperwork and labs prior to scheduling their appointment
  • Chapters 9, 10, 13 and 14 physicals require an escort
  • For flight and retirement physicals, please call the Puget Sound Military Appointment Center to schedule

Physical Health Assessment (PHA)

  • All soldiers must complete Part I of their PHA online prior to making an appointment
  • All PHA appointments will be scheduled at the Allen SCMH
  • Soldiers over the age of 40 will need labs and an ECG prior to their appointment

Vision

  • If you need glasses, please schedule an appointment with the Winder Optometry Clinic at 253-477-3788. They are open Monday-Friday, 7-11 a.m. and 1-3 p.m.
  • You will be notified by your battalion when your glasses are ready for pick-up
  • Some soldiers are eligible to receive free refractive (laser) eye surgery at the Madigan Warfighter Refractive Surgery Clinic. You can make an appointment directly with the clinic by calling 253-968-5516.

Hearing

  • To complete your annual hearing exam, please call the Madigan Hearing Program at 253-968-3431.
  • You can also call the Puget Sound Military Appointment Center and request a "well-hearing" exam.

History

The Allen Soldier-Centered Medical Home is named after Staff Sgt. Charles D. Allen, a medic assigned to 3rd Brigade, 2nd Infantry Division at Fort Lewis, Wash. He died from wounds sustained while conducting combat operations in Baghdad, Iraq on January 4, 2007.

Staff Sgt. Allen volunteered to replace a line medic on a patrol that day. As the patrol was returning to base, Staff Sgt. Allen was providing security from the rear hatch of his Stryker vehicle when he was killed by sniper fire.

Staff Sgt. Allen was the only combat medic killed in action with the 3rd Brigade, 2nd Infantry Division; he was posthumously promoted to staff sergeant, awarded the Bronze Star, Purple Heart, Meritorious Service Medal and Iraqi Campaign Medal.

While deployed on his second tour to Iraq in the latter part of 2006, Staff Sgt. Allen demonstrated great initiative as he excelled in various positions of increased responsibility. In Tai A'far, he made several improvements to his company's evacuation system, ensuring urgent medical care and evacuation was delivered on the battlefield. Exceeding the standard and expectations of his superiors, Staff Sgt. Allen was moved to Mosul and promoted to the role of platoon sergeant.

Charles was a dedicated father, husband, and friend. Medics who served with him at each assignment echoed the same remarks about his dedication to combat medicine, his love of Soldiering, his compassion toward his fellow teammates and his commitment to duty. There was never a doubt his purpose in life was to serve the country he loved for the family he cherished.

On October 8, 2019, a formal memorialization ceremony was held at the medical home dedicating it in honor of Staff Sgt. Allen with Allen’s family and close friends in attendance.
