Started in July 2010, Madigan Army Medical Center became the first medical treatment facility in the Department of Defense to teach a Fellowship in austere and wilderness medicine. This one-year Fellowship trains an individual to be a subject matter expert in all facets of wilderness and operational medicine. It provides wilderness medicine training equivalent to what would be obtained by completing a civilian wilderness medicine fellowship with the addition of training in all aspects of austere operational care, in particular Tactical Combat Casualty Care and extended care beyond the initial aspects of TCCC.

The Fellowship is one year in length and is open to military emergency medicine, internal medicine and family medicine physicians. A graduate from the Fellowship is expected upon completion of training to pursue an operational position where the skills learned can be put to use. As part of the training, the Fellow will become a certified AWLS instructor as well as obtaining the International Diploma of Mountain Medicine. The Fellow will be given opportunities to instruct in wilderness medicine both nationally and internationally. The Fellow will also be expected to acquire or further hone any pre-existing technical outdoors skills.
